Take advantage of the time you are with family and loved ones to share your wishes regarding medical care.
What is important to you?
If you become incapacitated and are unable to make decisions for yourself, who do you want to make them for you?
Have you shared your wishes with that individual?
Would you want to be placed on a ventilator?
Would you want a feeding tube?
Communicate your wishes with your loved ones, and then put it in writing with a Medical Power of Attorney and Living Will.
